Notices: please read carefully as this is a legally binding document

 

In consideration of being allowed to participate in the yoga classes and activities provided by Grow ‘n’ Shine Yoga, I, the undersigned Parent/Guardian, understand, acknowledge, and agree to the following:

​

  1. I am the legal parent or guardian of the aforementioned child, and I have the authority to sign this waiver on behalf of the participant.

  2. I am aware that yoga involves physical movements and that participation in yoga classes carries a risk of injury, both minor and severe.

  3. I acknowledge that it is my responsibility to ensure that my child is in suitable physical condition for participation in yoga activities. I have consulted with a healthcare professional if my child has any health concerns or medical conditions that might impact their participation.

  4. I understand that while Grow ‘n’ Shine Yoga takes measures to provide a safe environment and qualified instructors, there are inherent risks associated with physical activity, and I hereby assume all risks for my child's participation.

  5. I, on behalf of my child, release, waive, discharge, and hold harmless Grow ‘n’ Shine Yoga, its directors, instructors, employees, and volunteers from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my child's participation in the yoga classes or activities, including but not limited to injury, illness, or property damage.

  6. I consent to the use of any photographs, videos, or other media recordings taken during the yoga classes or events for promotional or educational purposes by Grow ‘n’ Shine Yoga. I understand that my child will not be identified by name, nor will any compensation be extended for such use.

 

I CONFIRM THAT I HAVE READ THIS WAIVER OF LIABILITY FORM AND FULLY UNDERSTAND ITS TERMS.

I ACCEPT THE RISKS AND BENEFITS OF COUNSELLING, THE NATURE AND LIMITS OF CONFIDENTIALITY, AND WHAT IS EXPECTED OF ME AS A CLIENT.
